Apply for a passport in Sac County. Passport applications are available Monday to Friday from 8:00 a.m. to 4:30 p.m. There is only one acceptance agent in our office so it is recommended to call 712-662-7789 before coming in.
You must apply in person using Form DS-11 if at least one of the following is true:
- You are applying for your first U.S. passport
- You are under age 16
- Your previous U.S. passport was issued when you were under age 16
- Your previous U.S. passport was lost, stolen, or damaged
- Your previous U.S. passport was issued more than 15 years ago
If none of the above statements apply to you, you may be eligible to renew using Form DS-82, allowing you to renew by mail and not be charged a processing fee.
Please note: If you are printing and completing your own forms, they must be printed single-sided.

Passports Available
Passport Book
Traditional book in which countries place their stamp. You must have a book when flying anywhere outside the United States.
Passport Card
Will fit in a wallet and allow land or sea passage to Mexico, Canada, the Caribbean and Bermuda. The card cannot be used for flying to a destination.
Adult Applicants (age 16 and older)
Passports valid for 10 years for applicants age 16 and older.
Passport Book
- Form: DS-11
Completed up to line 20 only, typed or legibly printed in CAPITAL LETTERS, using black ink. Application form must be signed in the presence of the Acceptance Clerk.
- Routine Application Fee: $130 check/money order only
Check or money order is required for the application fee. Make Payable to the "U.S. Department of State."
- Processing Fee: $35 check, cash, money order, or credit/debit card
This fee is paid separately. Payable to the "Sac County Recorder."
Passport Card
- Form: DS-11

Child Applicants (age 15 and younger)
Passports valid for 5 years for applicants aged 15 and younger.
Passport Book

- Both parents who are named on the applicant's birth certificate may apply for a minor under age 16. Both parents must sign the application.

Passport Card

Provide U.S. Citizenship Evidence
You must submit one of the following. Your evidence must be an original or certified copy. Photocopies and notarized copies are not acceptable.
- Fully-valid, undamaged U.S. passport (may be expired)
- U.S. birth certificate that meets the following requirements:
- Issued by the city, county, or state of birth
- Lists your full name, date of birth, and place of birth
- Lists your parent(s)' full names
- Has the date filed with registrar's office (must be within one year of birth)
- Has the registrar's signature
- Has the seal of the issuing authority
- Consular Report of Birth Abroad or Certification of Birth
- Certificate of Naturalization
- Certificate of Citizenship

Present ID
You (or your parent/guardian) must present one of the following IDs to the acceptance agent.
- In-state, fully-valid driver's license
- Fully-valid, undamaged U.S. passport (may be expired)
- Certificate of Naturalization or Citizenship (if the photo still looks like you)
- Government employee ID (city, county, state, or federal)
- U.S. military ID or military dependent ID
- Valid foreign passport
- Matricula Consular (Mexican Consular Identification, commonly used by the parent of a U.S. citizen child applicant)

Please note:
- Some states now issue digital ID documents (also known as mobile driver's licenses or mobile IDs). We cannot accept these digital IDs when you apply for your U.S. passport. You must continue to submit your physical photo ID and a photocopy of the ID.
- If you present an out-of-state ID, you must present an additional ID.
Provide a Photo
- One passport photo (Full face, 2"x2", white background)
- Office hours for taking passport photos are 8:00 a.m. to 4:00 p.m.
- Passport photos are $15 for a set of 2 prints
The photo fee is paid separately from the application fee. Cash, check money order or credit card will be accepted. Make checks payable to "Sac County Recorder."
